      A fall risk screening must assess whether an individual has experienced a fall or problems with gait or balance. A specific screening tool is not required for this measure, however potential screening tools include the Morse Fall Scale and the timed Get-Up-And-Go test. In order for a patient to be included in the Numerator, the assessment performed must be included in the “Falls Screening” value set (OID: 2.16.840.1.113883.3.464.1003.118.12.1028). To view value set contents, please visit the Value Set Authority Center (https://vsac.nlm.nih.gov/). Clinically equivalent services may be mapped to the contents of the "Falls Screening" value set. If mapping is done, you should maintain documentation in case of a CMS audit.
