      Thank you for your question about patient self-reporting for CMS131v9. Self-reported screenings are accepted if they are documented in the EHR using a SNOMEDCT code from the "Retinal or Dilated Eye Exam" value set. The EHR must reflect evidence of the retinal or dilated eye exam, the date of the exam, and the result. The documentation must also include evidence that the exam was performed by an eye care professional.

      The specifications for this measure does not appear to address this question:  If a patient self-reports the testing date and results for retinopathy, would that alone qualify to meet the Numerator for this measure?  Or is documentation of the eye exam (e.g. a scanned report) in the patient's EMR also required to ensure complete compliance with the numerator?
