      ​We checked with our pharmacy staff on our TAP. The above heparin dose is acceptable for this measure if given SQ for prophylaxis. However the value set you have listed 1) falls under Reason for no VTE prophylaxis and 2) refers to IV route dosing where our pharmacy TAP member stated that this would not be correct dosing.

      If you have further information you can provide we are happy to run it by our TAP members. Thank you.
