Need clarification on Numerator condition

      Timing relationships that appear as the last "bullet" in a logic block apply to the left-hand side of the immediately preceeding logic criteria. In this case, "Procedure, Performed: Treatment Adjustment by Protocol" and "Procedure, Performed: Clinical Pathway Protocol" are constrained to occur both after the unfractionated heparin medication order AND during the inpatient encounter. Please note, however, that "Occurrence A of Medication, Order: Unfractionated Heparin (route: 'Intravenous Route')" is not constrained by the last timing relationship in the logic block.
