Thank you for pointing out this issue. The duplicate logic was included in error and will be removed in the upcoming annual update. However, the duplicate logic will not necessarily be replaced with a block to address the ED visit timeframe. We are considering simplifying the numerator logic by removing criteria related to the last dose (most recent) parenteral anticoagulant administered during the ED visit. The rationale for this change is that while it is possible for a patient to be started on the parenteral anticoagulant in the ED, it is highly unlikely that this would also be their last dose of the medication. Therefore, we believe it is safe to assume that the last parenteral administration will occur during the inpatient encounter. We welcome your feedback on the removal of logic criteria addressing the possibility of the last parenteral anticoagulant dose being administered in the ED.

      Numerator states
      OR:
OR: "Medication, Administered not done: Medical Reason" for "Parenteral Anticoagulant "
OR: "Medication, Administered not done: Patient Refusal" for "Parenteral Anticoagulant "
<= 1 day(s) starts after start of (MOST RECENT:"Occurrence A of Medication, Administered: Parenteral Anticoagulant" during "Occurrence A of Encounter, Performed: Encounter Inpatient")

Here, the same 'OR' condition word to word is repeated twice in the Numerator population of the measure.
      Kindly explain the significance.
