ambiguous handling of null in `IndexOf` specification

the spec states

"If the list argument is null, the result is null."

however the [test](https://github.com/cqframework/clinical_quality_language/blob/master/Src/coffeescript/cql-execution/test/elm/list/test.coffee#L178) effectivly asserts that if either argument is null then the result is null
