In a previous question someone had posted, the resolution indicated patients from the surgical cohort would be excluded. I am not able to find where this is noted within the measure specification. Should this be accounted for prior to an organization submitted their hybrid measure files, or will CMS exclude the surgical cohort patients on their end? 

If vendors and hospitals are expected to exclude the surgical cohort prior to submission, could this be added to the specification?
