Please note that validating this work is highly time consuming and creates a lot of anxiety on quality managers that monitor these results. It also has created a lot of mistrust in the CMS process. Even though this ticket is addressing the mortality HSR, one must ask how can the Readmission measure be used in another program (TEAM) when there have been multiple reporting errors and inconsistencies this year?

Please fix the report. We need to know what our performance looked like for 2024-2025 which was the last voluntary period. Consider moving the Hybrid reporting deadline out giving participating hospitals enough time review results and make any necessary changes to their current files, or make the 2025-2026 reporting non-reportable (act like another voluntary reporting period).

The Aug 12, 2026 released Mortality HSR is showing failing labs for several of my 48 hospitals, it could be all of them but I have not completed my review. The labs have gone from high 80 and 90 percent performance to low teens (17%) and above. There are other inconsistencies in both the Readmission and Mortality HSRs, but the lab performance results for the Mortality HSR are by far the most consistent and impactful finding.
