Please note! The following two improvements have been implemented to the submission restrictions for voluntary reporting of hybrid HWR measure. This update is now live in the QualityNet submission system.

1) Patient records can be submitted with either a Health Insurance Claim Number (HICN) or a Medicare Beneficiary Identifier (MBI). (The system previously only accepted HICN.)

2) Only discharge date needs to fall within a reporting quarter for a record to be included in that quarter. Admission date no longer needs to fall within a reporting quarter for the record to be included in that quarter. (Previously, both admission date and discharge date needed to fall within the same reporting quarter for the record to be included in that quarter.)
