Hybrid eCQM data from multiple EHRs

      For the Hybrid eCQM submission period for FY 2026 (measurement period 7/1/23 - 6/30/24), our hospital has data between two EHRs. This is due to an EHR transition during the measurement period.  From 7/1/23 to 4/6/24, the hospital has hybrid data with one EHR. From 4/7/24 to 6/30/24, the hospital has hybrid data with another hospital. It would be infeasible for us to combine the data from the old EHR into the new EHR due to time and resource constraints. How should we proceed with this submission?  Should we submit Hybrid QRDA files from both the old/legacy EHR and the new EHR?
