Hi, In reviewing the 2022 - 2023 HSRs, we have several hospitals with missing QRDA encounters where the patient was admitted to rehab, psych or hospice unit. Our understanding from both the hybrid spec and FAQs (for eCQMs) attached, is that these patients do not get included/evaluated for the IP because they are excluded from IPPS.

Can you confirm if these same exclusions apply to the hybrid measure (makes sense that they would considering hybrid is a part of IPPS)?

If they are not to be included in the IP, why would the claim be on the HSR and why would it be counted in the performance threshold calculations?
