      LNC 75292-3 is now added to the Value Set for CMS 529v3 - Patient Reported Weight. On 11/29/2023, we received guidance from the QualityNet Service Center to not included any patient reported values or vitals signs in our submission data. We are requesting clarification on whether or not Patient Reported Values are now being accepted for this measure.

      Short Description: Patient Reported Weights
      Description: For the Hybrid Readmissions measure, can patient reported weight be used for the weight variable submission or does the weight need to be taken during the appropriate periods. We are particularly interested in this for patients who are admitted through the ED, as boarding rates can affect the ability to take a physical weight within the required time period.

      Thank you for your question. In generally, no, hospitals should not include patient-reported laboratory values and vital signs in their data submission. The measure is intended to include laboratory values and vital signs that are ordered and collected by the clinical hospital staff who treat the patient, as the aim of the risk-adjustment portion of the measures is to capture the status of patient's health upon arrival to the hospital.
