Do we need to submit patients who expire during their stay or are transferred to another acute care facility? A webinar I watched states that the Hybrid HWR measure includes only those who are discharged alive and not to another acute care facility. However, are these patients, who expire or are transferred, a part of the Hybrid HWM measure and need to be submitted? We have several Hospice patients in our initial population who expired during stay.
