I have patients who were direct admits from their provider's office, from the skilled nursing facility located within the hospital, and transferred from another hospital within our health system. In all these situations, is it okay to use the data for labs and VS as it is available in the EHR? Currently the report is mapping the earliest documentation in the 24 hours prior to admission, and this data is coming from when the patient was in these other locations.
