    • We are asking for the time frame to look for weight from initial patient contact, as opposed to just 24 hours prior or 2 hours after the inpatient admission is time stamped.

      In reviewing the results from our voluntary submission of the hybrid measure, CMS 529, we found that our hospitals were not meeting the requirement for submitting the patient weight on 90 percent of patients. The inpatient admission time stamp is not something that is recognized or observed by clinicians. Our hospital obtains vital signs and labs including weight upon initial patient contact typically emergency, clinic, pre-anesthesia, observation. The inpatient admission time stamp can occur near the beginning of the patient’s multi-day contact or later during the encounter period, which is greater than 24 hours after the initial weight is taken. Most vital signs and labs are repeated daily, these are being adequately reported for this measure. The weight, however, is not repeated and is not being counted. It would be a great burden on our clinicians to change practice to support this measure and obtain a weight within a time frame of 2 hours after the patient changes into inpatient status. We are asking for the time frame to look for weight from initial patient contact, as opposed to just 24 hours prior or 2 hours after the inpatient admission is time stamped.
