      Staff often do not know when a patient is switched from observation to inpatient. If a weight is obtained on a patient while in observation it should count. If I am understanding the measure correctly; weights must be documented within 24 hours following the start of inpatient encounter. If a patient has a weight obtained at 0700 while in observation status and is made inpatient that same day at 0730 the weight obtained 30 minutes earlier should be allowed. If I am understanding the logic correctly a weight obtained the next day at 0800 would be an outlier because it would fall outside the 24 hours from the timing of inpatient status.
