      I have a question about what we should pull for “Medication, Discharge” for the scenario below.

       

      Scenario:

      A patient was admitted to Inpatient on 06/01/2023 at 08:00.

      On 06/05/2023 at 10:00 a prescription was entered by the physician for “Percocet 5MG-325MG Oral Tablet” (Rx Norm Code 1049221).  

      • Dose: 1 Tablet
      • Frequency: Daily
      • Dispense Quantity: 25 Tablets
      • Days Supply: 25

      On 06/05/2023 at 11:00 the patient was Discharged from the hospital and went home.

      On 06/05/2023 at 12:00, the pharmacy contacted the physician and informed them that they did not have the medication, “Percocet 5MG-325MG Oral Tablet”, in stock.

      On 06/05/2023 at 12:15 the physician discontinued the prescription for “Percocet 5MG-325MG Oral Tablet” (Rx Norm Code 1049221).  

      On 06/05/2023 at 12:15 a prescription was entered by the physician for “Percocet 7.5MG-325MG Oral Tablet” (Rx Norm Code 1049225).  

      • Dose: 1 Tablet
      • Frequency: Daily
      • Dispense Quantity: 25 Tablets
      • Days Supply: 25

       

      Question:
