Hybrid measure logic misses labs due to patients on observation status

Our physicians are very careful about drawing a CBC and a CMP/BMP when admitting patients to the hospital. However, it is often appropriate to initially put the patient on observation status. In some cases, physicians decide that the patient's status should be changed to inpatient after more than 24 hours. In many cases, drawing a second CBC or BMP is not appropriate, and they don't order them. CMS has made cost reduction a priority. It would be inappropriate to draw labs, simply to conform with the time requirements of the hybrid measures. Consequently, the CCDE files that we submit are missing the results for the CBC and/or the BMP.

    • Request for change in logic for the hybrid measures CMS 529 and CMS 944
    • Could CMS alter the time frame for capturing the results for a CBC and CMP/BMP to also capture the lab results drawn when the patient initially is admitted to the hospital, for those cases the patient is initially put on observation status?
