Within an EHR there are the following times associated with a lab.  Which date/time should be used within Hybrid Measure reporting? 

• Time that the lab is collected (ex. blood drawn)
• Documentation time of when collection occured (I did the collection at 10:00 but didn't get around to chart until 10:05 when I backcharted that collection occurred) 
• Lab result date/time
• Doucmentation time of the lab result (lab result was complete at 11:40 which I jotted on a post-it.  Later at 11:50, I had time to go into the EHR and backchart that time.) 

Which time is the correct one to use for hybrid measure lab data? My assumption is the collection date time as the most clinically relevant but could not tell from the specification information.
