      The Hybrid-HWR measure requires a patient's weight to be documented within the time period of 24 hours prior, to 2 hours following inpatient admission. We are missing this window on many patients who are initially admitted to observation status, as they are usually weighed upon arrival to the ED or upon admission to observation. Observation stays can last up to 48 hours, and a typical patient does not require a daily weight.

      Please consider revising the specifications to provide a lookback period to the time of patient arrival for patients initially admitted to observation.
