We are trying to analyze our voluntary submission reports for the Hybrid HWR measure. The report easily identifies which cases had one or more of the 13 CCDE missing, thus contributing to not meeting the measure requirements. However, we had one entity that met the requirements for reporting vital signs and lab results but they failed the linking variables requirement. I am struggling to find information in the report on worksheet V. (Table 4--Discharge Level Information for the Hybrid HWR Measure) that indicates which encounters and/or linking variables were either missing or not matched to the CCDE from the EHR. Could you please direct me where this information can be found? What administrative claims data is used in comparison to the EHR file that is generated and submitted via HQR? It does not look like we are to generate anything in addition to the usual QRDA files for submission, so I am not understanding what the EHR information of CCN, HICN or MBI, DOB, Sex, admission date, and discharge date are being compared to and found as either not present or not a match?
