Linking variable-Northern Regional Hospital submitted the hybrid measure through Medisolv during the voluntarily submission period and received the workbook. I have found several discrepancies in what is showing in our EMR vs what is showing in Table 4 for the admission and discharge dates. Example:
ID Number 943 has 10/6/21 vs our EMR and the Medisolv dashboard has 10/7/22 as the discharge date of inpatient stay. Can you explain the difference between the two dates? I am assuming all the discrepancies in admission and discharges will cause a mismatch.
