      The FY 2020 Inpatient Prospective Payment System/Long-Term Care Hospital Prospective Payment System final rule requires that hospitals meet a series of requirements for the Hybrid HWR measure in order to be eligible to receive a full Medicare Annual Payment Update (APU) under the Hospital IQR Program. These requirements include 95% of linking variables, 90% of vital signs, and 90% of lab results for non-surgical patients.

      Received our HSR for the 2023 Hybrid HWR Measure from voluntary submission. Our hospital has discharges with failed linkage of claims and CCDE information. Are others having this same issue and please advise on troubleshooting?

      The QRDA file generated by our EHR only included patients that were 65 yrs and older per mapping specifications. Perhaps mapping is not aligned with what CMS is expecting...is CMS expecting all ages to be submitted via QRDA file and therefore planning on excluding on their end following submission?
