Hello,

A patient was seen in a provider office then went to ED and finally was admitted as an inpatient.  The first vital signs were taken at a physician office visit and are recorded in the shared electronic medical record although as part of a separate encounter (not part of the inpatient admission).  Would those vital signs be considered the first vital signs or do the vital signs need to be from an outpatient department or ED of the hospital?
