Bonnie QRDA Export of test patient for CMS9v5 contains conflicting Physical Exam Performed entries with the same "id" element. The entries are identical other than the "value" element. One entry has the information from the "code" element duplicated in the "value" element and the other [correctly] has result value in the "value" element.

An example of this issue can be found on the patient with last name of "PC5" and first name of "181" on the account "ron.peterson@ge.com".
