Overview 

Since 2015, CDC and the Office of the National Coordinator for Health IT (ONC) worked collaboratively to convert opioid clinical practice guidelines into standardized and shareable clinical decision support (CDS) interventions to be used in and by electronic health records (EHRs) to support appropriate prescribing. CDC released in March 2016 its Guideline for Prescribing Opioids for Chronic Pain based on the most recent scientific evidence (see link: http://www.cdc.gov/drugoverdose/prescribing/guideline.html). While prescription opioids can be an appropriate part of pain management, the CDC 2016 Prescribing Guideline aims to improve the safety of prescribing and reduce the harms associated with opioids, including opioid use disorder (OUD) and overdose. In 2022, CDC released a draft updated Clinical Practice Guideline for Prescribing Opioids (see link: https://www.federalregister.gov/documents/2022/02/10/2022-02802/proposed-2022-cdc-clinical-practice-guideline-for-prescribing-opioids). The clinical practice guideline updates and expands the 2016 CDC Guideline for Prescribing Opioids for Chronic Pain. Open-standard clinical decision support applications exist to assist health care providers in prescribing opioids within recommendations in the CDC Clinical Practice Guideline as well as state regulations. These tools typically can be used as either a free-standing system with connection to a state PDMP database, or as electronic health record (EHR)-embedded CDS using SMART on FHIR, enabling use in high-burden, resource-constrained clinics or large healthcare organizations, and a wide range of settings in between.  Building on an existing, successful electronic CDS tool for opioid prescribing, the purpose of this project is to expand the capacity for the application to support shared clinician-patient decision making and scale the implementation tool in resource-constrained clinical sites.
