I submitted CY 2024 one self-selected quarter (3Q CY2024) in the HQR portal. The QRDA files were accepted (no rejected files), however upon review of the files accuracy/outcome, I noticed that cases were counted twice per encounter (i.e. encounter from arrival to inpatient admission then inpatient admission to discharge). Our performance score based on our EHR (EPIC) is not the same as CMS performance score due to double counting. Would you please review CCN 140281 QRDA files submission for OP 40 STEMI to assists us in trouble shooting the issue. Thank you
