Note that the eCQM is not prescriptive regarding which eligible clinician providers must report on performance. However, should emergency medicine providers report on this measure, they must document a denominator exception when emergent care necessitates. In this measure, it explicitly indicates the denominator exception “Documentation of a medical reason(s) for not documenting, updating, or reviewing the patient’s current medications list (e.g., patient is in an urgent or emergent medical situation where time is of the essence and to delay treatment would jeopardize the patient's health status)”. While not all cases that present to the ED qualify for the trauma exception, it should be appropriately documented in the EHRs for vendor extraction when relevant.

Trying to ensure that we are mapping our medica record reports appropriately in our electronic healthcare record (EHR-Epic). Emergency providers are telling me they are not required to "review medications" on patients presenting in the Emergency Department because all Emergency patients are considered to be needing urgent or emergent care, and may not be able to provide medication history.
