Solution: | Thank you for your inquiry regarding CMS816v3, Hospital Harm – Severe Hypoglycemia. To meet the numerator a follow-up blood glucose test is not required within 5 minutes of the <40mg/dL reading. However, if a repeat blood glucose test is performed within the 5 minutes, and the result is >80mg/dL, then the encounter would not meet the numerator.
The purpose of the repeat test within 5 minutes is to eliminate false positives that can occur in point of care testing. The 5-minute timeframe extends from the start of the severe hypoglycemic test to the time of the repeat hypoglycemic test. The first scenario of a blood glucose < 40mg/dL AND a hypoglycemic medication administered in the 24h prior, but no repeat blood glucose meets the numerator. In the second scenario you describe, with a blood glucose result <40mg/dL, a hypoglycemic medication within the prior 24 hours and a repeat test result of <80mg/dL within 13 minutes of the initial test, the encounter also meets the numerator. |

Hello! I am needing add'l clarification re: the definition of the Hypoglycemic event, specifically as it relates to the repeat BG in a 5 min window following the initial BG < 40mg/dL. In order to meet the NUM is a repeat BG required in the 5mins after the initial BG <40mg/dL? Is a repeat BG required at all? Can you please clarify for me that if you have a BG < 40mg/dL AND a hypoglycemic med administered in the 24h prior, but no repeat BG at all, this would meet the NUM category? If I have a BG < 40mg/dL, hypoglycemic medication in the previous 24h, a repeat BG within 13m w a result < 80- this also meets NUM category?
