Solution: Thank you for your inquiry regarding CMS986, the Global Malnutrition Composite Score (GMCS). The request to add other patient status categories, outside the ED and Observation statuses has been previously evaluated through dialogues with a Technical Expert Panel and other experts in the quality measurement community. After a thorough analysis, including consideration of The Joint Commission standard that requires nutrition screening within 24 hours after an inpatient admission, completion of measure observations outside of the current timing constraints will not be included in this Annual Update cycle. Additionally, because of the impact on clinical changes that may require an inpatient admission, the measure intent encourages rescreening after admission. The measure steward plans to continue evaluation to ensure accurate capture of measure observations, alignment with national standards, and harmonization among other measures.

 Description   

In working through the new voluntary regulatory Global Malnutrition Composite Score (GMCS) process, we are having challenges with measure 1. Measure 1, as currently written, requires the malnutrition risk screen to be performed during the inpatient encounter including preceding observation or ED encounters. However, in our organization, we have identified that the risk screening for the patient encounter can take place pre-op, shortly prior to admission or during an outpatient procedure in which a patient has a complication and is admitted. Through our efforts and audits, we believe we are high performing in the area of GMCS. However, if the measure continues to be defined with these strict rules, we do not believe we will be able to meet this measure. We respectfully request that CMS consider adding this type of category or a timeframe prior to admission for allowance to meet this criteria.
