Solution: Thank you for your follow-up question, specific to CMS153v12, Chlamydia Screening in Women. Although the 2024 value set Chlamydia Screening (OID 2.16.840.1.113883.3.464.1003.110.11.1129) does not contain LOINC 21613-5, the 2025 value set does contain the specified value. Specific to 2024 reporting, if mapping is conducted, you should maintain documentation in case of a CMS audit.

 Description   

When the 2024 eCQM measure value sets were released EHR vendors removed 43305-5 results as a numerator qualifier because of this. The LOINC code was then added back in the latest version v2.77 released on 2024-05-02. This is impacting providers performance outcomes across the board. LabCrop & Quest use this LOINC code when reporting results for Chlamydia trachomatis rRNA [Presence] in Specimen by NAA with probe detection, which is a valid chlamydia screening test result. 

 

Please address - fix and provide clarification EHR’s vendor’s that this is a valid LOINC code and HAS TO BE ADDED BACK ASAP, otherwise regulatory reporting results will be significantly impacted with incorrect reporting results. There was a previous case created on 04/15/24 and resolved on 04/24/24, with the following solution: Thank you for your inquiry for CMS153v12: Chlamydia Screening in Women. Clinically equivalent services may be mapped to the codes used in "Chlamydia Screening" (2.16.840.1.113883.3.464.1003.110.12.1052) to satisfy the numerator requirement. Thank you for your follow-up comment for CMS153v12. Value sets and codes for 2024 eCQM reporting once finalized cannot be amended. The values LOINC 21613-5 & LOINC 43304-5 are found in the 2024 value set (for 2025 reporting period). However, clinically equivalent services may be mapped to the codes used in a measure's value sets to satisfy a value set requirement for the 2024 reporting period. The codes (LOINC 6349-5 & LOINC 6355-2) codes can be mapped to clinically equivalent concepts used in the measure's value sets to satisfy the requirement found within the "Chlamydia Screening" (2.16.840.1.113883.3.464.1003.110.12.1052) value set. You can reference the applicable codes containing these value sets on the Value Set Authority Center (VSAC) at https://vsac.nlm.nih.gov/. Click on the “Search Value Sets” tab and enter the value set ID to review codes included in the respective value set. We are unable to provide specific guidance related to the mapping of codes. We recommend you consult with your EHR vendor and clinical partners.  If mapping is done, documentation should be retained in case of a CMS audit.

Mayo Clinic is observing the same issue severely impacting performance.  LOINC codes impacted include 21613-5, 43304-5, 6349-5, 6355-2.
