Solution: | Thank you for your question regarding STK-05, Antithrombotic Therapy By End of Hospital Day 2. The antithrombotic value sets (2.16.840.1.113762.1.4.1110.62) consist of RxNorm concepts that represent a dose formulation to meet intent and are designed to broadly to meet various clinical scenarios. You can reference the applicable codes containing these value sets on the Value Set Authority Center (VSAC) at https://vsac.nlm.nih.gov/. Click on the “Search Value Sets” tab and enter the value set ID to review codes included in the respective value set. Administering medication through a PEG tube meets the intent of the measure because an oral dose form would still be used. |

Impact: | We are not getting credit for qualifying antithrombotic medications ordered in an oral form unless they are administered by mouth. If the oral medication is administered via a nasogastric or other enteral feeding tube, they are in the denominator but do not meet the numerator. |
Description |
STK-05 Inclusion Criteria states, Includes concepts that represent a medication that is an antithrombotic medication, including oral, rectal, and injectable forms. If the oral form of antithrombotic medication is ordered to be administered via enteral route (NG tube, PEG tube), and is administered as such, does this meet requirements to be included in the numerator? I can find no guidance specifying routes" that are included or excluded, only forms of medications. |
