By documenting this quality action (an attestation) a medication list was documented using all recourses available to them at the time of the encounter If the EP is unable to document the patients list of current medications due to a Medical Reason: Patient is in an urgent or emergent medical situation where time is of the essence and to delay treatment would jeopardize the patient’s health status the patient meets an exception. The medical reasons for not documenting the meds could be due to these snomed codes: > 183932001- Procedure contraindicated (situation) > 397745006- Medical Contraindication (finding) > 407563006- Treatment not tolerated (situation) |

The Medical or Other Reason not done SNOMED-CT value set (2.16.840.1.113883.3.600.1.1502)inlcudes the following SNOMED options: In eCQM 68,the Procedure, Performed not done appears to be "Current Medications Documented. In order to meet the denominator exceptions the EP must report that the Procedure -"Current Medications Documented" was not performed due to one of the "Medical or Other Reason not done" SNOMED codes listed above. If the Procedure in question is an "ACT whose immediate and primary outcome (post condition) is the alteration of the physical condition of the subject..."(QDM) then the SNOMED options seem appropriate. If the Procedure the EP is expected to perform is the act of documenting the patients list of current medications these SNOMED codes do not appear to be appropriate options for explaining why the list of current medications was not documented. Can you please verify that I have interpreted the denominator Exceptions correctly and if so, is there any expectation that the value set will be modified in the yearly update? |
