Solution: Thank you for your question about the denominator exception “Diagnosis of Allergy to Egg” for CMS147v9 “Preventive Care and Screening: Influenza Immunization,” which is for the 2020 performance period. The measure does not require an annual diagnosis of egg allergy. The Overlaps operator asserts that there exists some point in time at which the event on the left-hand side (the egg allergy diagnosis) is effective at the same time as the event on the right-hand side (influenza season). In cases where an event lacks a recorded end date/time, the Overlaps operator will interpret the missing end date/time as ongoing. The start of the egg allergy does not have to be after the beginning of the influenza season. Instead, the prevalence period for the egg allergy must continue after the influenza season to meet the criteria for the denominator exception. In your example, if the start date (2016) is on or before the end of influenza season (March 31, 2020) and there is no end date, this diagnosis will meet the criteria for the denominator exception. If there is an end date and it is on or before the end of the influenza season (March 31, 2020), this will not meet the criteria for the denominator exception.

Impact: I am working on eCQM 147v9 and I am questioning the part of the CQL. Can you clarify the intention of the measure?

One of the denominator exceptions is Diagnosis of Allergy to Egg. The CQL is represented as follows.

CQL Line from eCQM:
["Diagnosis": "Allergy to Eggs"] EggAllergy
       where EggAllergy.prevalencePeriod overlaps after "Influenza Season"

Interpreted as:
       start of flu season (Oct 2020) <= start of Allergy (2016)---False
      and start of Allergy (2016) <= end of flu season(Apr 2021)--- True

If I am understanding “overlaps after” correctly, the start of the egg allergy would have to be after the beginning of the flu season.

This seems at odds with what I understand about egg allergy. We have a situation where an egg allergy was documented in 2016 and fails our unit test.

Does CMS want us to update the egg allergy every year during the flu season?
