Institution/Name: Southcoast Hospitals Group
Guidance required: Please
Solution: Correct, the Obstetric patients are excluded from the measure. I have attached the value sets which contain the specific diagnoses excluded in the measure. These can be accessed on https://vsac.nlm.nih.gov/.

•"Diagnosis: Obstetrics" using "Obstetrics Grouping Value Set (2.16.840.1.113883.3.117.1.7.1.263)"
•"Diagnosis: Obstetrics VTE" using "Obstetrics VTE Grouping Value Set (2.16.840.1.113883.3.117.1.7.1.264)"
Impact: In reviewing the VTE 1 measure "population" description , it states : include patients > than 18 years of age and i.e. and without the diagnosis of VTE or obstetrics. I just want need to confirm with our analysts that the obstetrical population is not in the VTE 1 measure ...correct? Thank you

Thank you for the additional detail.

Code Z.33.1 is a secondary code only for use when the pregnancy is in no way complicating the reason for visit. This code is not appropriate to include as an exclusion, as in these cases the primary diagnosis may be a condition that places the patient at risk for VTE.

The remaining Z.34 supervision codes make sense to add to the Obstetrics exclusion, as they indicate a patient had a pregnancy encounter, and these codes are always first-listed and are not to be used with any other code from the OB chapter.

We intend to add the Z34 codes you suggest. We will not add Z.33.1 for the reason stated.

Hello,
Thank you for a quick response back ! The below normal "OB" codes are not listed as any of the OB exclusion codes for VTE1. Could you please confirm these OB codes are included or excluded from this measure? From a previous response, we realize "OB" codes are excluded but upon further investigation it appears not all OB codes are on this list. Thank you !

These are the codes that are not in the Obstetrics code list for VTE-1

Z331 Pregnant state, incidental
Z3400 Encounter for supervision of normal first pregnancy, unspecified trimester
Z3401 Encounter for supervision of normal first pregnancy, first trimester
Z3402 Encounter for supervision of normal first pregnancy, second trimester
Z3403 Encounter for supervision of normal first pregnancy, third trimester
Z3480 Encounter for supervision of other normal pregnancy, unspecified trimester

Z3481 Encounter for supervision of other normal pregnancy, first trimester
Z3482 Encounter for supervision of other normal pregnancy, second trimester
Z3483 Encounter for supervision of other normal pregnancy, third trimester
Z3490 Encounter for supervision of normal pregnancy, unspecified, unspecified trimester
Z3491 Encounter for supervision of normal pregnancy, unspecified, first trimester
Z3492 Encounter for supervision of normal pregnancy, unspecified, second trimester
Z3493 Encounter for supervision of normal pregnancy, unspecified, third trimester

If you could be more specific regarding what codes are not listed, we would be happy to evaluate the codes and address them with the Technical advisory panel.

Thank you for your response below, but I have one more question.

From the below, you confirm this measurement VTE-1 excludes the obstetrical patients ....but the normal obstetrical ICD pregnancy codes are not listed within the excluded value sets. Could you please let me know the reasoning for this or if they are truly included or excluded. We are just trying to complete our reporting systems ....

Thank you
