Solution: The only way to identify patients who have delivered is with ICD-10 procedure codes. ALL deliveries MUST be coded to one of the procedure codes in the Delivery Procedures ICD-10-PCS value set (2.16.840.1.113762.1.4.1045.58). The only exception would be an extremely rare case when the patient delivered precipitously with no hands on guidance (manual assistance) what so ever. In other words code 10E0XZZ from the Delivery Procedures ICD-10-PCS value set (2.16.840.1.113762.1.4.1045.58) will be used frequently and applies to a case when the delivery is a manually-assisted, vaginal delivery defined as assisting the passage of the products of conception from the genital canal.

Impact: I would like to follow up with the following question that was resolved and closed last month:

2015 EH Measures:
CMS113v4/NQF0469
Impact: Several value sets for CMS113 only contain code system ICD9CM. We are concerned as of Oct. 1st 2015 ICD10CM will go into effect. These value sets are evaluated in the IPP.

While the solution for this issue is logical, there are concerns that we will no longer pull ICD-9 codes for diagnoses and will therefore have only one means in which to capture this data which is through the procedure code. Previously we were able to capture data through one of two ways, the procedure code or the diagnosis code, which allowed for the increased ability to meet the measure if one of the two codes were entered incorrectly. Now, if a procedure code is entered incorrectly, there is no means of capturing the measure through the diagnosis code which will lead to higher fallouts and an overall lower patient volume for the measure. I feel there will be a much lower volume of patients in this measure at our facilities if no ICD-10 codes are implemented for this measure going forward. Please reconsider the solution for this issue as we will no longer see ICD-9 codes from our facilities and this will extremely limit our ability to capture patients into this measure.

Solution:
The concept of delivery is represented as a procedure in SNOMED CT and ICD-10-PCS and as a diagnosis in ICD-9. The Delivery Procedures and Diagnosis value sets are not grouped as they represent different concepts and grouping would suggest equivalence. The IPP logic of “Procedure, Performed: Delivery Procedures” uses a grouping value set that contains SNOMEDCT and ICD-10-PCS codes for delivery procedures. The remaining “Diagnosis, Active” logic statements use only ICD-9 value sets, but represent the same concept- delivery.

To summarize, eCQM reports will not be negatively impacted upon transition to ICD-10, as the ICD-10 representation of delivery is already included in the measure.
