Stage 2: PATIENT REMINDERS - 2 or more office visits. See attached pdf Question: This is a unique patient level measure. The measure requires 2 or more office visits with ‘the’ EP within 24 months before the beginning of the EHR reporting period. Does ‘the’ EP mean that the same EP has to see the patient at least twice during the 24 months in order to meet denominator? What happens when two different EPs have seen the patient in 24 months prior to the start of the reporting period? This is our interpretation, currently the credit is being given to the provider whom the patient has had his latest encounter with out of the two. Example: This is how the measure is currently configured to work. • Encounter1#: ‘Provider A sees the patient on 12/31/2013 Reminders are sent within the Q4 reporting period by patient preference. In this case, we are giving credit to Provider B, since he was the most recent EP who rendered services to the patient. Your assistance on this is much appreciated. Regards, |
