| Solution: | The <measure-strata-num> field is to indicate when a measure has more than one performance rate to submit. There is a small subset of the 2014 PQRS measures that require multiple performance rates to be submitted and are outlined in the measure specification. Many of the measures only require one performance rate to be submitted and for those cases, the <measure-strata-num> should be '1'. |

There is a requirement in the PQRS 2014 specification which talks about <measure-strata-num> as one of the data elements to be present in the XML file for submission. It talks about the sub-stratum number defined in the measure specification. None of the PQRS 2014 measures we are using has any reference to this field in the measure specs. What would be the value that gets displayed in this case? Currently we have defaulted it to ‘1’ as defined in the Registry Reporting XML specification requirements |

We had validated our measure group XMLs some time in December 2014 on the SEVT Tool and it had passed validation. We tried validating the same files over the SEVT Tool on 2/4/2015 and received the following error codes for <pqrs-measure-details> tag (Error codes – 1123/1126) under the Group XML. The individual XML seemed to pass the validation successfully. The format for <pqrs-measure-details> tag is exactly same for both individual and group XMLs for measure #122. I really don’t understand why would it fail for one and pass for the other now when both were successfully getting validated in Dec 2014? We are doing measure #122 and it is the only measure with multiple performance rates and everything in the file has been implemented as per the PQRS 2014 Registry XML specifications. We have included a <pqrs-measure-details> tag for each strata. The eligible instances is same for all three strata’s the other five tags are different for each depending on what quality action was performed. The data within the tag <measure-strata-num> is 1, 2 and 3 like in the example below. <pqrs-measure> |
