Thank you for your question related to CMS1017, Hospital Harm – Falls with Injury. For unrelated injuries and falls, the specifications state that patients with a fall present on admission are excluded from both the numerator and denominator. Please see the logic below:

Encounter where a Fall and Moderate Injury Occurred

o "Encounter where a Fall Occurred" FallOccurred
      o where exists FallOccurred.diagnoses ModerateInjury
o where ModerateInjury.code in "Moderate Injuries"
o and ( ModerateInjury.presentOnAdmissionIndicator in "Not Present On Admission or Documentation Insufficient to Determine"
      o or ModerateInjury.presentOnAdmissionIndicator is null
      o )

The logic only accepts a moderate-injury diagnosis when the POA indicator is either “Not Present on Admission or Documentation Insufficient to Determine” or null. So in your scenario, if the patient was admitted because of the moderate injury and that injury has a POA indicator of present on admission, it would fail this condition and would not, by itself, make the fall a numerator event.

If the encounter is being included in the numerator despite the injury being unrelated to the fall, we would recommend reviewing how the fall event and associated diagnoses are being documented and mapped in the source data to confirm that the injury diagnosis is not being linked to the fall event.

    • Numerator patient being included for a injury not related to the fall.

An inpatient was admitted for a moderate injury.  They were being treated for the injury and had a fall while in the hospital.  The admitted injury was not related to a fall.   The patient is still being reported as a numerator due to the Adverse Fall event and the moderate injury coding although injury was not related to the fall.  Can you please advise how the current specifications address if you have a valid fall and a valid injury included in the value set that are pulling into the report but are not related?  Thank you.
