Thank you for your question. We are interpreting this as a question for CMS506v8, Safe Use of Opioids—Concurrent Prescribing.

For CMS506v8, which has a measurement period of January 1 through December 31, 2026, the measure is scored as a proportion. The denominator begins with qualifying inpatient hospitalizations for patients age 18 or older who have at least one opioid and/or benzodiazepine medication at discharge, subject to the specified denominator exclusions. The numerator includes those hospitalizations in which the patient has two or more opioids, or an opioid and a benzodiazepine, at discharge. Decreased scores indicate improvement.

This question is outside the scope of the CMS506v8 measure specification. The specification defines the clinical measure logic and confirms that the measure is scored as a proportion; however, it does not define the structure or interpretation of external public-use datasets or research files. Questions regarding the April 2026 dataset—including the meaning of the score and sample fields, reporting status, and data availability—should be directed to the organization that published the dataset (for example, the CMS Provider Data Catalog or the Hospital IQR/QualityNet support team, depending on the source of the file). You can submit your question to the QualityNet Question and Answer Tool (https://cmsqualitysupport.servicenowservices.com/qnet_qa).

The measure team is not able to interpret dataset-specific publication fields.

We are trying to use the April 2026 ECQI data on hospital harm - safe use of opioids. Only some hospitals have "score" data at all, and if they do, it is a 0 or 1. Most guidance suggests that the score should be a proportion. Given that mandatory reporting is starting in 2027, is the current data just indicating whether the hospital is intending to report something (yes or no) this year? Is the data useable for analyses at this point? What does the "sample" column indicate?

      Clarity on current scoring methods
