Thank you for your questionregarding CMS104 and CMS71. These measures are episode-based measures. An episode is defined as each inpatient hospitalization or encounter that ends during the measurement period.

For the eCQMs the logic utilizes the "Medication, Discharge" data type. When utilizing the "Medication, Discharge" data type, the logic will look for the authored time stamp of a medication on a patient discharge medication list.

Information on this data type is available on page 41 of the QDM 5.6 Publication (https://ecqi.healthit.gov/sites/default/files/QDM-v5.6-508.pdf), available on the eCQI Resource Center.

If the medications are on the discharge medication list, and the discharge medication list was authored during the inpatient encounter, then the patients will meet the numerator.

Per TJC for the chart abstracted measures, STK-2 and STK-3, addendums placed within 30 days on the discharge summary can be accepted for documented exceptions for the measure. However, the electronic measures do not accept any addendums placed by the provider if they are after the discharge date.

The electronic measures eSTK-2 and eSTK-3 vary from the chart abstracted measure in that documented exceptions for antithrombotic therapy or anticoagulant therapy are not acceptable even if they are placed within 30 days of the discharge date, and are addendums to the initial discharge summary. Is this an intentional change or can this be reviewed to allow consistency between the two types of abstractions?
