Brief description of measure
CMS156: Use of High-Risk Medications in Older Adults:
Percentage of patients 65 years of age and older who were ordered at least two high-risk medications from the same drug class. Three rates are reported.
1. Percentage of patients 65 years of age and older who were ordered at least two high-risk medications from the same drug class.
2. Percentage of patients 65 years of age and older who were ordered at least two high-risk medications from the same drug class, except for appropriate diagnoses.
3. Total rate (the sum of the two numerators divided by the denominator, deduplicating for patients in both numerators).
Description of issue
The measure developer received the following prescription scenario as an inquiry about whether the scenario would meet the measure's criteria for "two prescription orders on different days": "a provider places two orders for the same high risk medications on the same date but sets the start of the second order to be a day in the future (e.g., 30, 60, 90 days later); while the two orders were placed on the same date, the start date of the second order is not until a date in the future, which could make it look like two orders on different days, depending on how an EHR handles the data; it is essentially planning a refill".
This scenario suggests clinician intent for repeated use of high-risk drugs and would therefore meet measure intent.
The measure developer would like to solicit implementer feedback on how often this scenario may occur in clinical practice and if the proposed change - using the relevantPeriod of the ordered medication - would capture the scenario as intended.
