We plan to move forward with the proposed change during this annual update.
Brief Description of Measure:
CMS147: Preventive Care and Screening: Influenza Immunization:
Percentage of patients aged 6 months and older seen for a visit between October 1 and March 31 who received an influenza immunization OR who reported previous receipt of an influenza immunization
Description of Issue:
The current immunization administration period in the measure is from August 1st of the year prior to the measurement period to March 31st of the measurement period. We propose aligning the immunization administration time period to July 1st of the year prior to the measurement period to June 30th of the measurement period when influenza vaccines are licensed for the current flu season (which is still October 1st of the year prior to the measurement period to March 31st of the measurement period).
The licensing for each vaccine product is determined by the FDA annually. The FDA typically licenses influenza vaccines from July 1st to June 30th; so although the vaccinations may not start to be widely distributed until later in the summer, stakeholder consensus recommends the measure align with licensure standards. https://www.fda.gov/vaccines-blood-biologics/vaccines/vaccines-licensed-use-united-states
Proposed Solution:
Current defintion header language:
Previous Receipt - receipt of the current season's influenza immunization from another provider OR from same provider prior to the visit to which the measure is applied (typically, prior vaccination would include influenza vaccine given since August 1st)
Proposed definition header update:
Previous Receipt - receipt of the current season's influenza immunization from another provider OR from same provider prior to the visit to which the measure is applied (typically, prior vaccination would include influenza vaccine given since July 1st)
Current Denominator header:
Equals Initial Population and seen for a visit between October 1 and March 31
Proposed Denominator header:
Equals Initial Population and seen for a visit between October 1 of the year prior to the measurement period and March 31 of the measurement period
Current Numerator header:
Patients who received an influenza immunization OR who reported previous receipt of an influenza immunization
Proposed Numerator header:
Patients who received an influenza immunization OR who reported previous receipt of an influenza immunization between July 1 of the year prior to the measurement period to June 30 of the measurement period
Current logic:
Influenza Immunization Administered
["Immunization, Administered": "Influenza Vaccine"] AdministeredFluVaccine
where AdministeredFluVaccine.relevantDatetime during "Influenza Season Including August and September of the Prior Year"
Proposed logic:
Influenza Immunization Administered
["Immunization, Administered": "Influenza Vaccine"] AdministeredFluVaccine
where AdministeredFluVaccine.relevantDatetimeduring day of " Between July 1 of the Year Prior to the Measurement Period and June 30 of the Measurement Period "
Current definition name and logic:
"Influenza Season Including August and September of the Prior Year"
Interval[start of "Measurement Period" - 5 months, start of "Measurement Period" + 3 months )
Proposed definition name and logic:
"Between July 1 of the Year Prior to the Measurement Period and June 30 of the Measurement Period"
Interval[start of "Measurement Period" - 7 months, start of "Measurement Period" + 6 months )
Current logic:
Influenza Vaccination Procedure
["Procedure, Performed": "Influenza Vaccination"] FluVaccination
where Global."NormalizeInterval" ( FluVaccination.relevantDatetime, FluVaccination.relevantPeriod ) during "Influenza Season Including August and September of the Prior Year"
Proposed logic:
Influenza Vaccination Procedure
["Procedure, Performed": "Influenza Vaccination"] FluVaccination
where Global."NormalizeInterval" ( FluVaccination.relevantDatetime, FluVaccination.relevantPeriod ) during day of " Between July 1 of the Year Prior to the Measurement Period and June 30 of the Measurement Period"
Rationale for Change:
The FDA typically licenses influenza vaccines from July 1 to June 30; so although the vaccinations may not start to be widely distributed until later in the summer, stakeholder consensus determined that we should align with licensure standards.
