Remove the word 'ever' from the description to align with measure intent

      Brief Description of Measure: 

      ​CMS127: Pneumococcal Vaccination Status for Older Adults:
      Percentage of patients 65 years of age and older who have ever received a pneumococcal vaccine.

      Description of Issue:

      The numerator header language reads: Patients who received a pneumococcal vaccination on or after their 60th birthday and before the end of the measurement period; or had an adverse reaction to the vaccine before the end of the measurement period
      However, the measure description reads: Percentage of patients 66 years of age and older who have ever received a pneumococcal vaccine. The word "ever" suggests that the vaccine can be taken at any time during a patient's history. To avoid confusion, we will take out the word "ever".

      After:

      Rationale for Change:

      We propose revising the measure description to reflect the current specification which is to assess vaccines received at the age of 60 and older. The goal of this change is to provide clarification on the measure description. **
