We recommend reaching out to your EHR or quality reporting vendor to find out how they are mapping your local medications to the standardized codes found in the value set.     • Capturing medications at Discharge due to missing drug component

      On the discharge medications:

      The requirement to capture these medications at discharge is to have the four drug components such as Route of administration, strength dose, strength dose unit, drug form. "Drug form" for physicians is sometimes not completed due to the physician not knowing what form the medication is in the formulary. i.e. tab vs capsule. This has led to some medications not being captured. Is it necessary to have drug form as a requirement in order to qualify for capturing discharge medications?
