Thank you for your question. You are correct that assessments performed during observation, a status prior to admission, will not be counted in the numerator.

We agree that it would be important not to order additional consults to meet the measures.

The challenge of accurately representing observation affects many inpatient measures. In 2015, the Change Review Process evaluated potential solutions to address capturing observation data for eCQMs, and concluded that limitations to the Quality Data Model and eCQM expression language prevent efficient representation of observation care. Plans to address observation are on hold until a new expression language, CQL, is used in eCQMs.

Please review CQM-1608 for a summary and for updates on this topic: https://oncprojectracking.healthit.gov/support/browse/CQM-1608

    • Meeting the Measure of STK 10 while in Observation status prior to Inpatient Status

On Stroke 10 - patients who are in observation status prior to having an order for inpatient status are receiving Consults for Speech or Physical Therapy. These Evaluations are being completed prior to the patient's actual status of "inpatient" during the patient's encounter and are failing this measure. - Yet the intent of the measure is being met. The timing is a mismatch between the evaluation completed and the inpatient order - therefore it is not being captured. Is there a solution being worked on to correct this? It would be important to not order additional "consults" as "inpatients" just to meet these measures.
