      NCQA would like additional feedback on incorporating patient self-report for labs into certain eMeasures. In particular, we think that for the following lab and measure combinations, there will not be enough data available to warrant adding a patient provider communication data element to the measure.

      Sometimes when a clinician is taking a patient history, the patient will report labs that have been performed in the past. For example, a patient may inform the provider that she had a pap smear. Those could count as numerator events as long as the minimum amount of information needed to ensure the service happened in the required time period is present. Sometimes the measure requires the result as well as date when the test was performed. Many times this information is not recorded in discrete fields, rather it is captured in text notes or other formats that are not usable.
